"action_default_fallback" for all stories' forms

I am getting “action_default_fallback” for all the stories after form is executed. Earlier everything was working fine but not after asking for questions for a form, it is not performing specified action and is displaying “action_default_fallback” msg at the end. I have tried deleting and retraining models but still same problem. Also after “action_default_fallback”, my slot values is resetting to None.

Any suggestions would be helpful. Thanks in advance!!

@Bobby Did you tried delete the older models and try trained model?

@nik202 , I have moved my stories to rules and now they are working. Strange!!!
