How do i extract entities value ad filled it up inside the slots of a form

As described in the title, i wanna extract the entities value in the slots and set it inside the form slots.

For example, Japanese restaurant in [Kuala Lumpur]{“entity”: “city”, “value”: “Kuala Lumpur”} both values will be auto filled into my form required slot (cuisine and city).
