FormAction failed to use the utterance

Hi,

I’m started working in Rasa for passed 2 months, now using FormAction to fill the slot. I have issue in that, Formpolicy failed to use the utterance template if the slot filled. I have trained the rasa with utterance template once slot is filled. But once the training is over, i checked whether its working or not, slots are filling properly but there is no utterance template Kindly Assist me in this issue

Found the solution for this issue, used the utter_ask_{slot value} as the question to fill the slot
