Form not activated even though all the rules are followed

I have created a bot which logs in using a form.

This form worked like a charm.

Now I m trying add one more form in similar fashion now what so ever I do the form is not getting activated on send of intent xx.

I have added rules ,domain,customaction and stories as well as told by rasa docs .But even now 1st login form is good second form is not working not sure what to do

Run the bot with the --debug option and you can review the log messages to see what is going wrong with the prediction. You can also share a link to your repo.
