Unable read data using conditional forms in rasa 2

Can anybody help me to create a conditional form in rasa 2. For example, If i have a question, Did you exercise yesterday? if answer is yes, I want to ask what type of exercise? else, I want to skip type of exercise question.

How can i do that…anybody please help me ASAP.
